APP Primary Care - Chronic Kidney Disease

This module series, which encompasses a wide variety of commonly encountered clinical topics, are specifically designed for the new primary care APP. Key areas of emphasis are on the diagnosis and management of acute and chronic conditions, preventative screenings & vaccinations, and referral indications. Best practices and common evidence-based principles are embedded into these courses. All course presenters are APPs within Sentara, across a spectrum of primary care and specialty areas. After completion of the series, it is expected that the new primary care APP will have a proficient level of clinical knowledge to guide their care of patients, improving quality, safety, efficiency, and patient experience.

1. Understand the incidence, impacts, and risk factors associated with chronic kidney disease on the population.
2. Identify the stages of chronic kidney disease, lab monitoring, and proper management, including in the setting of hypertension and diabetes.
3. Recognize acute kidney injury, including GFR changes, etiologies, patient presentation, and indications for hospitalization.
4. Know when it is appropriate to refer patients to nephrology.
