Overview
HCC Coding and Quality Education sessions help providers apply new HCC and quality guidance to everyday practice. They focus on updating existing codes to more specific HCC codes, meeting quality metrics, and using strategies that support complete and compliant documentation. Providers also learn how to teach these coding and documentation standards within their own practices to support accurate reporting, appropriate compensation, and overall system goals.

Objectives
At the conclusion of this activity, learners will be able to:

  1. Discuss YTD HCC performance and Epic HCC POC workflows post 4/2026 Upgrade
  2. Differentiate appropriate coding and documentation for Asthma vs Reactive airway disease.
  3. Identify and appropriately capture common Pulmonary HCCs across the 3 Risk models
  4. Describe coding and documenting Pulmonary HCC codes within case scenarios/questions.

Accreditation

Sentara is accredited by the Southern States CME Collaborative to provide continuing medical education for physicians.

Physicians: Sentara designates this Live Activity for a maximum of 0.75 AMA PRA Category 1 Credit™.  Physicians should claim only the credit commensurate with the extent of their participation in the activity.
